Erectile Dysfunction and Venous Leak: Modern Diagnostic and Endovascular Treatment Approach

Erectile dysfunction (ED), also known as impotence, is a very common condition and in many patients it is related to vascular causes. Contrary to popular belief, the underlying problem is often venous insufficiency (venous leak) rather than arterial insufficiency.

Today, ED related to venous leak can be treated with surgical or endovascular methods. Although no treatment guarantees 100% success, carefully selected patients can achieve very good outcomes with modern techniques. One of these methods is venous embolization, a minimally invasive procedure aimed at closing the leaking veins.

📌 What’s new in current practice?

With recent scientific advancements, we now enhance treatment success by improving pre-procedural diagnosis and planning.

🔍 CT Cavernosography plays a key role in this process:

• It maps the penile venous anatomy in detail
• Identifies the exact location and extent of venous leakage
• Is performed while the patient is awake
• Takes approximately 10–15 minutes
• Uses low radiation doses, making it a safe procedure

Thanks to this technique, the venous system can be visualized three-dimensionally, from multiple angles (front, side, and top), allowing precise identification of the main leakage points, such as the deep dorsal penile vein, as well as associated superficial veins.

📍 Using these detailed findings, embolization is then performed under angiographic and cavernosographic guidance, significantly improving the ability to predict treatment outcomes and tailor the procedure to the individual patient.

Advanced imaging combined with targeted endovascular therapy allows a more precise, personalized, and effective treatment approach for venogenic erectile dysfunction.
